Job Summary:

Flexible, part-time position!

Working under the leadership of the Service and Support Administration Manager and direct supervision of the Family and Children First Council Manager, the Community Health Worker (CHW) is responsible for helping clients and their families navigate and access community services and adopt healthy behaviors. Through home visits, the CHW assists clients in overcoming barriers to health, social services, education, and employment. The CHW also provides social support, informal counseling, and advocates for individual and community health needs. The CHW expertly navigates the health and human services systems to bridge the gap between communities and health and social service systems.

Qualifications:

• Completion of High School Diploma/GED required, formal training and/or related course work
from a college or other education institution preferred
• Must hold and maintain a current, valid driver license
• Must maintain insurability with automobile insurance carriers
• Must acquire and maintain Board approved first aid and CPR certification
• Must pass BCI/FBI background check
• Knowledge of community resources, experience providing direct service to clients/families and
strong interview/information gathering skills preferred
• Must successfully complete Community Health Worker training and obtain certification through
the Board of Nursing within 12-months of hire date
